Assess, supervises and provides skilled nursing
• Licensed Vocational Nurse (LVN)
Works under supervision of Registered Nurse.
• Home Health Aides (HHA)
Provides direct patient care under the supervision of RN to include personal hygiene, ambulation, exercise, safe transfers and other supportive tasks as assigned
• Physical / Occupational Therpist (PT & OT)
Guides and instructs patients in prescribed therapeutic self-care and creative activities that are directed toward individual physical and mental independence.
• Medical Social Worker (MSW)
Provides counseling, community awareness, financial resources, transportation, and medical assistance
• Speech Language Pathologist (SLP)
Assesses and evaluates communication disorders, develop plan of care and treat the patient to level of competence
• Personal Assistance Services (PAS):
Assist client in personal care and housekeeping tasks.
